Your data is any information about you. We collect and keep some data about you so that we can provide the healthcare services to you that you expect from your GP. We keep this data in a secure environment, and we only keep what we need for as long as we need to.
Some of the types of personal data we keep are: Name, Address, Date of Birth, Gender, Phone Number, Email address, Family members, Insurance information, Medical card information, PPSN Numbers. We also keep Special Category Data, which is more sensitive and so is under tighter control. This is your Health Data, and can include medical notes, health issues, medications, test results, reports from other health professionals, referral letters, medico-legal data, and other health related correspondence.
We are committed to protecting the security and privacy of your data. All members of our staff are aware of their obligations to protect your data, and have signed confidentiality agreements. We only use your data in ways that you would expect us to in the provision of GP and other Medical Services. This is your data. You have the right to ensure that the information is correct, and the right to access the information, to stop us processing it, to have it erased in certain circumstances, and to have it transferred to you or to another person securely.
